I trained and worked as a medical doctor, and it was in the clinic—listening to patients describe pain, fear, hope, and uncertainty—that I first noticed something striking. Their reasoning did not unfold independently of language; it was a superstructure of language. The OSV framework grew directly out of that insight: that grammar is not a symbolic overlay on thought, but the machinery through which thought organizes events, possibilities, and world-states.
