I practiced medicine for over fifteen years, where bedside listening revealed two recurring narrative orientations: patients who return to a single center of concern (radial) and those who move laterally across topics (tangential). Combining clinical observation with formal linguistics, I developed a domain-based account linking conversational projection to spatial grammar, yielding testable predictions and practical tools for clinicians and researchers.
