Introduction: The aim of this study was to design and use a sensor suitable for tropical countries to investigate the association between objective wear time and treatment efficacy of Twin Block appliance. Methods: Twin block appliance incorporating new electronic timing device was delivered to thirty children in the age group of 11 to 14 years. Participants were instructed to wear the appliance full time and recalled at 3 to 4 week intervals. Cephalograms and study models were taken at baseline and after six months of appliance therapy. Results: Data from 27 of the 30 participants were analyzed. Wide variation in daily wear time observed amongst participants. Based on time of wear, they were divided into full time (FT >17 hours/day) and part time (PT <12 hours/day) wear groups. Mean wear durations were 20.86 hours in the FT group and 9.55 hours in the PT wear group. In skeletal changes, ANB angle were reduced by 2.690 in FT group and 1.330 in PT group, for Pg/OLp (FT, 2.22 mm; PT, 0.44mm) which was statistically significant. In dental changes, overjet were reduced by 3.91 mm and 2 mm in FT and PT wear group respectively, difference was statistically significant. Conclusion: There were significant differences in many dental and skeletal changes achieved with PT and FT wear of a Twin-block appliance. Maximum treatment effect of Twin block appliance was found with full time wear over 6 months of treatment period.
